Family Day Care Home Provider Request Form for CACFP Non-congregate Feeding Waiver due to COVID-19

On March 20, 2020, USDA issued a waiver that will better enable providers to offer meals to enrolled children while minimizing potential exposure to COVID-19. This waiver takes effect March 20, 2020, and will remain in effect through June 30, 2020, or until expiration of the federally declared public health emergency, whichever is earlier.

This CACFP Non-congregate Feeding Waiver allows meals/snacks to be consumed off-site by your enrolled children only; you may not provide meals for other children living in their households (e.g. siblings).
